What is a Self-Declaration Document?
The self-declaration document may be issued to an individual who wishes to care for 5 or fewer children, under the age of 12. No more than 3 of those children may be under the age of 24 months. A provider must also count thier own children who are under the age of 12 in their total number.
Self-declaration documentation and licensing are similar, but a self-declaration document is not a child care license.
